Appointments
The office is open Monday through Friday. Since each physician has a
different schedule, patients are seen by appointment only.
Each physician has his or her own patients. Should you have a problem
that requires immediate attention and your physician is not available,
you may be asked to see the physician on call. If the problem is not
serious and you would prefer to see your own physician, we will be happy
to schedule an appointment at the earliest possible date.
If it is necessary to cancel your appointment, please do so within 24 hours of appointment to avoid a $25 charge and to allow someone else to use that appointment time.
Forms
There will be a $5 charge for each form completed. Payment must accompany
the form. All forms must be received in our office at least 10 working
days before the filing date so they may be properly processed.
Getting the Most from Your Visit
1. Bring a list of questions with you. Go over the entire list with your doctor.
2. Bring a list of any symptoms or problems your child/you have been having. It is most useful to share this information with your doctor at the beginning of the visit.
3. Bring a note pad with you to the doctor visit. It is often helpful to take notes on the following:
-The answers your dictor gives to your questions.
-Instructions the doctor gives for caring for your child/yourself at home.
-Your doctor's explanations about your childs/your medical condition.
-How medicines work and how they should be taken.
No Show/Failure to Cancel Policy.
There will be a $25 charge for No-show appointments and those appointments not canceled within 24 hours. (This will not be billed to insurance company. It is the patient/family responsibility.)
Other Services
To provide you convenience and accessibility, we have the following services
available at our facilities:
- Or Pro - Orthotics (Braces) and Prosthetics
(Artificial Limbs)
- Physical Therapy/Occupational Therapy
- X-ray
- Laboratory
These services are also available at our office located in the South
Point Office Building in Centerville. Our staff will offer you
the option to receive these services at the most convenient location.
The services listed above will be billed separately from services received
through the Orthopaedic Center. Our staff at each office will be pleased
to provide you with answers to any questions regarding these procedures.
Parking
The Children’s Medical Center charges a $1 fee for parking regardless
of the time spent.
Prescriptions and Refills
Prescriptions and refills are issued during office hours only. During
the evenings and weekends it is difficult for the doctor to determine if
a prescription or refill is indicated because the medical record is
not available. This means that the doctor will not routinely write
prescriptions or refills during the evenings or weekends. If you need
to request medication, you should call early in the day and at least
48 hours in advance so we can best take care of your needs. We will
also ask for your pharmacy telephone number, so please have it available
when you call.
Telephone Calls
The physicians at the Orthopaedic Center have instructed their assistants
to handle all incoming telephone calls. This allows the physician to
attend to his or her patients with a minimum of interruptions. Should
your call concern an emergency, the assistant will immediately contact
the doctor. The doctor will instruct the assistant, who will return
your call as soon as possible.
